Request Nutrition Kit Supplies and/or Lessons — LPS Non-Title Schools

Important: The form below is ONLY for Lincoln Public Schools Non-Title (49% or less free or reduced lunch count) elementary schools in Lincoln, Nebraska. Kits are already located in each qualifying school. Restocking supplies are delivered annually, based on request. Please fill out the form below if you wish to have nutrition kit supplies delivered to your school and/or if you wish to have an Extension staff member teach a lesson in your classrooms. Each school and grade (K-5) may request ONE in-classroom lesson from an Extension staff member — options include: hand washing, physical activity or food experience (snack).
